Key Responsibilities
* Drive client-centric care management, empowering individuals and their support systems to access comprehensive services and achieve optimal outcomes.
* Foster strategic partnerships with service providers, leveraging linkages to enhance care coordination and ensure seamless transitions.
* Spearhead community outreach efforts, promoting client engagement, medical appointment compliance, and self-sufficiency through targeted support and empowerment strategies.
* Lead client communication initiatives, ensuring timely updates, record accuracy, and exceptional service satisfaction.
* Manage precise documentation, verifying timesheets and maintaining up-to-date electronic records in compliance with agency policies and regulatory requirements.
* Architect application management processes, ensuring timely coordination and monitoring caregiver compliance to drive successful outcomes.
* Champion incident reporting and customer service excellence, providing prompt notification to supervisors and delivering responsive, patient-centered support.
* Collaborate with key stakeholders, including the Department of Social Services and Managed Care contracts, to ensure regulatory adherence and optimal service delivery.
